The aim of phase two of the outcomes co-design process is to develop a suite of tools and processes for capturing evidence of outcome.
At this stage feedback from participants is divided between (a) wanting a standard tool for each service type and (b) wanting a suite of tools to choose from, depending on local context, age of client etc.
Both options would be accompanied by clear counting rules and/or guidelines on administration and contextualisation of the tool used.
